Intake paperwork will be sent to you via email once I confirm your appointment and must be completed 48 hours prior to intake appointment or your appointment will need to be rescheduled.

This allows me time to review your completed paperwork, note any questions I have, and request additional information - if necessary - from your paperwork.

No matter your experience level or length with technology platforms, completing paperwork online can be confusing.

Technology Supports I Utilize in My Practice

  • I utilize Zoom for all virtual sessions with the accessibility features of Live Captions, alternative camera backgrounds, and text-based messaging during sessions.

    I utilize these and am open to structuring sessions in manners that increase and facilitate accessibility for folks desiring additional access points to support.

  • BluePrint Health is available to all clients to complete regular assessments, keep notes for discussions at future sessions, and track the impact of therapy over time in a data-driven manner.

    During the first few sessions, clients collaborate on choosing which check-ins are enabled and measurements are assessed.

  • Upheal AI allows me to offer higher-quality, more focused therapy sessions where documentation is supported by AI.

    This frees me to focus on our work together and offer note summaries to clients if they’d like to receive a brief summary of each session between sessions.

  • Simple Practice Electronic Health Record system maintains my documentation and appointment scheduling.

    Clients are generally scheduled in series of 6 sessions at the start of our work together. Over time, client can transition to every other week sessions and to monthly sessions or ‘as needed’.

    Sessions can be scheduled online and clients can connect via Secure Message between sessions via SimplePractice or Google Voice.

Out of Network Benefits Guides and Links

For clients outside of Oregon and those wishing to utilize Health Insurance as a form of payment via SuperBill.

  • What are Out of Network Benefits and What is a SuperBill?

    More therapists than ever are choosing to forgo contracting with Private Insurance Companies in the interest of maintaining more control over their practice, income, and to provide options for out of pocket payment, therapy without needing to offer diagnoses or assessment, or flexibility in quantity or length of therapy sessions.

    Some therapists have the option to give you something called a Superbill. This is essentially a fancy receipt with all the information your insurance company will need to file a claim to utilize Out of Network Benefits to get reimbursed for some - or even all - of the expense of therapy.

  • Verify Your Out of Network Insurance Benefits via Reimbursify

    Kimberly Louvin, LCSW is out-of-network (OON) for many health insurance plans. Many individuals have health insurance plans that will reimburse a portion of your costs for OON treatments. Future Clients can verify their Out of Network Benefits via Reimbursify and submit claims directly via Reimbursify with SuperBills Complete a quick form to authorize this practice to obtain your specific coverage & deductible information for you.

  • Submit Out of Network Claim via Reimbursify

    When utilizing Out of Network benefits you’ll likely need to pay your therapist the entire session fee at the time of service, and depending on your specific plan, your insurance company will mail you a check to reimburse a portion of that cost.

For Self-Pay clients, I offer Standard Rate

& PWYC Sliding Scale Therapy.

In cases where we both agree we are a good client-therapist fit, but my standard rate is out of financial capacity, I am generally able to offer to meet you where you are in ability to pay. I do not require any financial documentation from you, and we will handle all discussion of fees with just a human-to-human conversation.

My Standard rate:

$200/45 minute session

$250/60 minute session

PWYC Sliding Scale : $5-249

Pay What You Can (PWYC) sliding scale therapy allows me to ensure that my private practice aligns with my values to offer a practice to clients that allows each of us to show up to session as present and grounded as possible.

Oregon Clients utilizing Commercial Health Insurance, be sure you are set up in Headway for Billing.

If you are not already set up with Headway for Insurance Billing for Commercial Insurance payments, please visit my profile link below and establish a Headway Account from the email you should receive after our Consult Call.

Headway is a third-party company I work with to be able to offer clients the ability to utilize Insurance as a form of payment to increase accessibility of therapy services.

Clients will utilize SimplePractice for Telehealth Appointments, 1:1 communication, and will be billed via Headway’s billing services.

I am in-network with the following Insurance Companies via Headway: Aetna, Cigna, Oscar, Oxford, Blue Cross Blue Shield of Oregon and Massachusetts, and United Healthcare.

If you are seeking out of pocket sliding scale rate services under $100, you will need to join Open Path Collective as a member before our first session.

My Open Path per session fees are $70 for individuals and $80 for therapy with more than one person.

Once you’ve completed your membership application, you’ll submit your one-time $59 membership fee to Open Path via debit, credit, or check.

Next, Open Path will send you an Open Path Member ID email, which you can forward to me before our first session.

From there you’re all set to discuss your per-session fee with your therapist and schedule your first session.

If you are currently without financial means, I do reserve a single spot for a lower fee. Contact me to see if this spot is available.

  • What will our first appointment be like?

    Feel free to review this section of my website and email me with any questions you have about therapy prior to our intake appointment.

    Intake appointments generally include me asking a lot of questions and gathering information to inform how to best support you, assess if I need to gather any additional resources, and help you feel comfortable working together.

    We will also set a per session fee and schedule out at least 3 initial appointments for both of our calendars.

  • Payment and Fees

    During our intake appointment, we will establish the fee per session rate based on the length of sessions (30-90 minutes), your financial situation, Open Path Membership, and additional information you offer.

    Fees are charged immediately after each session and future appointments may be cancelled or adjusted for nonpayment.
